The main difference between metabolic alkalosis and respiratory alkalosis generally lies in their underlying mechanisms. Respiratory alkalosis is caused by respiratory factors, while metabolic alkalosis results from disturbances in the body's metabolism. Additionally, changes observed in arterial blood gas analysis may differ: although pH levels can exceed 7.45 in both conditions, respiratory alkalosis primarily manifests as a decreased partial pressure of carbon dioxide (PaCO₂), whereas metabolic alkalosis is characterized by increased base excess (BE) and elevated bicarbonate (HCO₃⁻) levels.
