Congenital hydrocephalus can be divided into non-surgical and surgical treatments. Mild hydrocephalus is generally managed with non-surgical treatment, primarily involving dehydration therapy, mannitol, and systemic supportive care. Surgical treatment is indicated for cases with high intraventricular pressure, a cerebrospinal fluid column exceeding 250 mm, or failure of non-surgical treatment. In severe hydrocephalus—such as when head circumference exceeds 50 cm, cerebral cortex thickness due to atrophy is less than 1 cm, and severe functional deficits or malformations are present—the outcome of surgical intervention is usually poor.
