The symptoms of epididymitis mainly fall into two categories: acute epididymitis and chronic epididymitis. 1. **Acute epididymitis**: Symptoms are usually severe and include sudden high fever, scrotal distension and pain with a sensation of heaviness, which worsens when standing or walking. The affected epididymis becomes swollen and tender upon palpation. When the inflammation is extensive, both the epididymis and testicle may swell, making their boundaries indistinct upon touch. Additionally, the spermatic cord may thicken and be tender. 2. **Chronic epididymitis**: Symptoms are generally milder but more common. Patients often experience dull pain and a sensation of fullness or heaviness in the affected side of the scrotum, with pain possibly radiating to the lower abdomen and ipsilateral groin. The area around the epididymis may exhibit varying degrees of enlargement and firmness, accompanied by mild tenderness. Chronic epididymitis often develops from inadequately treated acute cases, although it may also occur without a clearly defined acute phase, frequently following chronic prostatitis or trauma.
