Chronic anal fissure refers to a condition where the anal fissure has persisted for more than six weeks, with slow or incomplete wound healing. Symptoms last for a prolonged period, tend to recur frequently, and usually cannot resolve spontaneously. Symptoms of chronic anal fissure are typically more frequent and severe, and patients may experience persistent anal pain. Additionally, chronic anal fissure may be accompanied by blood in the stool, although the amount of bleeding is usually small, often only evident as blood stains on toilet paper.

Patients with chronic anal fissure are advised to improve their dietary habits, increase dietary fiber intake, maintain regular bowel movements, reduce straining during defecation, and use topical medications as needed.
