My son is three months old, and he starts coughing as soon as he begins breastfeeding. We are unsure what might be causing this. Doctor, could you please explain why an infant would cough while feeding?

Dr. Zhou Xiaofeng

Babies coughing immediately after feeding may be caused by improper feeding, swallowing air, respiratory tract infections, allergic factors, or tracheoesophageal fistula.

1. Improper feeding: Incorrect feeding posture, such as lying flat while feeding, may lead to poor swallowing and milk entering the airway, causing coughing.

2. Swallowing air: During feeding, babies may swallow excessive air, leading to gastric discomfort and subsequent coughing. This situation is often accompanied by hiccups or spitting up milk.

3. Respiratory tract infection: After infection with pathogenic microorganisms, infants may develop respiratory diseases. The respiratory mucosa becomes sensitive, and the milk may irritate the area when passing through the respiratory tract during feeding, resulting in coughing.

4. Allergic factors: Babies may be allergic to certain components in formula or breast milk, or be sensitive to allergens such as dust mites or animal dander in the feeding environment, leading to increased airway reactivity and coughing symptoms.

5. Tracheoesophageal fistula: This is a congenital developmental disorder where a fistula exists between the esophagus and trachea. During feeding, milk may flow into the trachea through the fistula, causing reflexive coughing.

Parents should carefully observe the baby's symptoms and take appropriate measures for relief and prevention. If symptoms persist or worsen, timely medical consultation and examination are necessary.
