Do patients with appendicitis experience lower back pain?
Appendicitis is also a common condition. During its onset, patients often experience migratory abdominal pain; in severe cases—when the pain is intense—other areas of the body may also be affected. So, can appendicitis cause lower back pain?

Do patients with appendicitis experience low back soreness or pain?
Generally speaking, only a very small proportion of patients with appendicitis report low back soreness or pain. Notably, this symptom is most commonly observed in cases involving a retrocecal appendix (i.e., an appendix located behind the cecum). In addition to low back pain, these patients typically present with migratory right lower quadrant abdominal pain, as well as other clinical manifestations such as nausea, vomiting, abdominal distension, fatigue, and fever.
At this point, patients are advised to seek prompt medical attention. Antibiotic therapy—preferably broad-spectrum antibiotics—is recommended for infection control. Additionally, preoperative evaluations and preparations should be completed thoroughly, followed by timely surgical intervention. Surgical options include laparoscopic appendectomy or conventional open appendectomy, with the choice depending on the patient’s clinical condition and socioeconomic considerations.
Following appendectomy, low back soreness typically subsides gradually, and abdominal pain resolves completely. Postoperatively, patients should continue antibiotic therapy, maintain fluid and electrolyte balance, and mobilize early (i.e., get out of bed and walk regularly) to promote recovery. The vast majority of patients have an excellent prognosis after appendectomy.
In severe cases of appendicitis, marked abdominal pain occurs. This may stretch surrounding nerves, potentially triggering referred pain in the lumbar or dorsal regions. Under physician guidance, pain relief may be achieved through pharmacologic analgesia or surgical treatment. Furthermore, to prevent recurrence of appendicitis, strict attention to dietary habits and adoption of a healthy lifestyle are essential.
