Introduction
Erythromycin ointment, a macrolide antibiotic ointment, can be used to treat acne. However, due to the emergence of clinical antibiotic resistance, its efficacy in treating acne is limited. When acne primarily presents as inflammatory papules—i.e., moderate acne—topical application of compound chloramphenicol alcohol solution is recommended, as it provides effective anti-inflammatory and antimicrobial activity.

Erythromycin ointment contains erythromycin, wool fat, and petrolatum, and appears as a white to yellowish soft ointment. It is indicated for purulent skin diseases; it may also be applied to small-area burns or infected skin ulcers. Additionally, it can be used topically for vulvar infections presenting with erythematous rashes and pruritus.

Is Erythromycin Ointment Effective for Acne?

Erythromycin ointment belongs to the macrolide class of topical antibiotics and may be used for acne treatment. However, due to increasing clinical antibiotic resistance, its efficacy against acne is generally limited. When acne primarily presents as inflammatory papules—i.e., moderate acne—topical application of compound chloramphenicol alcohol solution is often preferred, as it provides effective anti-inflammatory and antimicrobial action. In more severe cases, oral antibiotics may be added; commonly prescribed agents include clarithromycin extended-release tablets or tetracycline-class antibiotics.

For severe acne characterized by furuncles, cysts, and pustules, early treatment may involve low-dose systemic corticosteroids to control inflammation, followed by oral or topical antibiotics. In refractory cases, isotretinoin may be required. Erythromycin ointment is also indicated for impetigo, folliculitis, and other superficial skin infections. If clinical response is suboptimal, alternative topical agents such as mupirocin ointment (e.g., Bactroban®) may be considered, alongside appropriate oral antibiotic therapy.
