Gallbladder polyps are relatively common in today’s population. A widespread misconception exists regarding gallbladder polyps: many people believe that as polyps grow larger, the risk of malignant transformation increases—thus warranting early intervention.

Do gallbladder polyps pose a problem?

If significant changes in a gallbladder polyp are observed, surgical removal is generally recommended. The need for surgery indicates that the polyp has become clinically significant. Regular monitoring via B-mode ultrasound is advised. Should a polyp exceed 1 cm in diameter, immediate surgical resection is indicated.

Gallbladder polyps themselves are typically asymptomatic and non-painful. However, if pain develops, concomitant cholecystitis or possible malignant transformation must be considered, and surgical treatment may become necessary. Surgical indications for gallbladder polyps include: coexisting cholecystitis; rapid polyp growth; polyp size exceeding 1.1 cm; or high clinical suspicion of malignancy.

For instance, if a polyp grows from 0.4 cm to 2.8 cm within one year, this represents rapid growth—and surgical intervention is strongly recommended. Gallbladder polyps are an umbrella term for various benign mucosal elevations of the gallbladder wall. Most patients exhibit symptoms similar to those of chronic cholecystitis, primarily mild discomfort in the right upper quadrant. When associated with gallstones, biliary colic may occur. Notably, a substantial proportion of individuals remain entirely asymptomatic and only discover the polyp incidentally during routine health examinations—often leading to delayed diagnosis.

Solitary gallbladder polyps with broad-based attachment occupy a relatively large portion of the gallbladder lumen and significantly impair gallbladder function; prompt intervention is therefore advisable.
