What Are the Clinical Manifestations of Advanced-Stage Hepatocellular Carcinoma?
Hepatocellular carcinoma (HCC): Symptoms and clinical manifestations of HCC in its advanced stage include the following:
1. Hepatic pain: Often persistent dull or distending pain in the right upper quadrant; occasionally severe pain may occur, radiating to the right shoulder—prompt analgesic therapy is essential.
2. Hepatomegaly: Over 90% of patients exhibit progressive liver enlargement due to tumor growth.
3. Jaundice: Typically appears in late-stage disease, resulting from hepatocyte damage and/or biliary obstruction caused by tumor compression or invasion of the hilar bile ducts. This leads to impaired bile excretion, causing yellow discoloration of the sclera and skin, often accompanied by pruritus.
4. Other symptoms: Include hepatic ascites (abdominal distension), anorexia, fever (sometimes exceeding 39°C), and frequent chills—fever often results from systemic absorption of necrotic tumor tissue into the bloodstream. Gastrointestinal dysfunction may also occur, leading to indigestion. Additionally, coagulopathy in advanced HCC may cause cutaneous or upper gastrointestinal bleeding.

HCC often presents insidiously, with nonspecific or absent symptoms in early stages. By the time symptoms become apparent, the disease has frequently progressed to intermediate or advanced stages. HCC commonly develops on a background of cirrhosis or may first manifest as metastatic lesions, both scenarios increasing the risk of clinical misdiagnosis or delayed diagnosis. Hepatic pain is the most common symptom in intermediate-to-advanced HCC.
