A microscopic examination of the discharge (e.g., potassium hydroxide [KOH] preparation) can confirm fungal infection. If diagnosed with a fungal infection, topical miconazole cream is recommended, along with cleansing the scrotal skin using a sodium bicarbonate solution. 2. **Scabies infestation** (caused by *Sarcoptes scabiei* mites) may also cause intense itching—often worsening at night. Diagnosis requires examination for characteristic burrows in the scrotal skin or direct identification of mites. Upon confirmation, thorough local hygiene measures should be implemented, and topical sulfur ointment should be applied for treatment.
