Conservative treatment is the first-line approach for thumb tenosynovitis. Surgery should only be considered if conservative measures fail. During acute exacerbations, thumb movement should be minimized or avoided altogether. Temporary immobilization using an external splint is recommended to prevent worsening of the condition. Additionally, microwave physical therapy and blood-activating, stasis-resolving medications may be used; nonsteroidal anti-inflammatory drugs (NSAIDs) can be administered either orally or topically.

Patients with tenosynovitis are often unaware of their condition initially. As time progresses, the pain associated with tenosynovitis gradually worsens. So, how is thumb tenosynovitis treated?

How Is Thumb Tenosynovitis Treated?

The first-line treatment for thumb tenosynovitis is conservative management. Surgery is considered only if conservative measures fail. During acute flare-ups, thumb movement should be minimized or avoided entirely. Temporary immobilization using an external splint is recommended to prevent disease progression. Concurrently, microwave physical therapy and blood-activating, stasis-resolving medications may be employed. Nonsteroidal anti-inflammatory drugs (NSAIDs) can be administered either orally or topically. If strict conservative treatment proves ineffective after 2–3 weeks, corticosteroid injection—typically triamcinolone acetonide—into the tendon sheath may be considered.

Thumb tenosynovitis commonly results from chronic overuse, localized traction, friction, or cold exposure. Therefore, the most critical preventive measure for patients is avoiding contact with cold water; warm water is strongly recommended—especially during winter months. Additionally, local irritants should be minimized: activities such as daily vegetable washing, dishwashing, and laundry may aggravate symptoms and should thus be reduced.

If repeated corticosteroid injections (typically two to three attempts) yield unsatisfactory results, surgical intervention may be considered as a definitive option. Surgery can fully resolve thumb tenosynovitis and prevent recurrence. We hope this information proves helpful to you.
