Excessive exercise may lead to muscle fatigue, resulting in muscle soreness; some patients may experience pain on both sides of the buttocks. Cold exposure or chilling can cause local tissue ischemia and hypoxia, leading to soreness in the buttock muscles.

The causes of soreness, swelling, and pain on both sides of the buttocks include excessive exercise, cold exposure, hip osteoarthritis, lumbar spine disorders, and soft tissue muscle injuries.

1. Excessive exercise: Overexertion may lead to muscle fatigue, resulting in muscle soreness. Some individuals may experience pain specifically on both sides of the buttocks.

2. Cold exposure: Exposure to cold or chilling temperatures can cause local tissue ischemia and hypoxia, leading to soreness and pain in the muscles on both sides of the buttocks.

3. Hip osteoarthritis: Osteoarthritis affecting both hip joints is particularly common among middle-aged and elderly individuals. Degeneration and overgrowth of hip joint cartilage, along with narrowing of the joint space, can trigger aseptic inflammation in both hip joints. This results in feelings of soreness and discomfort, as well as pain on both sides. Since the hip joints are located close to the muscles on both sides of the buttocks, inflammatory processes spreading to these areas may also cause soreness and discomfort in the buttocks.

4. Lumbar spine disorders: Patients may develop herniated discs that compress nerves and blood vessels extending to the buttocks and lower limbs. This compression can lead to nerve inflammation, degeneration, or even damage, manifesting as soreness, swelling, pain, or numbness in the buttocks and even the lower extremities.

5. Soft tissue muscle injury: Muscle and soft tissue injuries on both sides of the buttocks are commonly seen in young and middle-aged individuals, especially those who frequently engage in physical activities such as running, hiking, or mountaineering. The buttock muscles have relatively large volume and play an important role in various lower limb movements. During exercise, excessive strain or accidental injury may result in muscle damage or overuse, causing soreness and pain on both sides of the buttocks.
