Introduction
Lower back pain, abdominal discomfort, and the urge to defecate may be caused by physiological factors or pathological conditions such as pelvic inflammatory disease or colitis. In individuals during menstruation, these symptoms—such as lower back pain, abdominal fullness and pain, and the urge to defecate—may occur due to the gradual shedding of the endometrium. This is a normal physiological phenomenon that does not require special treatment and can be relieved by keeping the body warm.

Lower back pain, lower abdominal discomfort, and the urge to defecate may be caused by physiological factors or pathological conditions such as pelvic inflammatory disease or colitis. The specific analysis is as follows:

I. Physiological Factors

Individuals during menstruation may experience lower back pain, lower abdominal bloating, and a sensation of needing to defecate due to the gradual shedding of the endometrium. This is a normal physiological phenomenon that does not require special treatment. Symptoms can be relieved by keeping the body warm.

II. Pathological Factors

1. Pelvic Inflammatory Disease (PID)
Infections caused by organisms such as *Chlamydia trachomatis*, *Escherichia coli*, or *Neisseria gonorrhoeae* may lead to pelvic inflammatory disease. Under stimulation by inflammatory factors, the pelvic region may produce the aforementioned symptoms. Treatment may include medications such as metronidazole tablets or levofloxacin hydrochloride capsules, taken as directed by a physician.

2. Colitis
Colitis may result from fungal or bacterial infections. Inflammation in the colon can lead to ulceration, causing these symptoms. Medications such as mesalazine sustained-release granules or azathioprine tablets may be used under medical guidance.

In addition, lower back pain, lower abdominal discomfort, and the urge to defecate could also be caused by other conditions such as proctitis, duodenitis, or colorectal cancer. Patients are advised to actively cooperate with medical treatment.
