Introduction
In most cases, lower abdominal pain occurs during the premenstrual phase, often accompanied by referred low back aching—symptoms resulting from endometrial shedding. When this occurs, it is important to keep warm, avoid cold exposure, refrain from consuming cold foods, and ensure adequate rest without overexertion.

Menstruation serves as a monthly indicator of a woman’s overall health. Whether menstruation arrives on schedule, and whether menstrual flow volume and color are normal, are concerns many women monitor each month. Some women also experience abdominal pain and lower back discomfort before their period begins—symptoms that closely resemble early pregnancy signs. As a result, many young women struggle to distinguish between premenstrual lower back pain and early pregnancy-related discomfort. Let’s explore these differences together.

Are Lower Abdominal Pain and Lower Back Pain Typical Premenstrual Symptoms?

In most cases, mild lower abdominal pain occurs during the premenstrual phase, often accompanied by referred lower back soreness. These symptoms arise from the natural shedding of the endometrium. When experiencing such discomfort, it is important to keep warm, avoid cold exposure, refrain from consuming cold foods, and ensure adequate rest—avoiding excessive physical exertion.

Exposure to cold during this time may trigger uterine contractions and spasms, potentially leading to dysmenorrhea (painful menstruation). Rest remains essential, and overexertion should be avoided, as it may increase menstrual flow and cause excessive bleeding. Additionally, maintaining emotional well-being and minimizing stress is highly recommended.

Both early pregnancy and the premenstrual phase can cause breast tenderness and nipple sensitivity. However, in early pregnancy, women often report sharp, stabbing, or itchy sensations in the breasts, along with noticeable breast swelling. Additional signs include darkening of the areolae and prominent subcutaneous venous patterns on the chest. In contrast, premenstrual breast changes typically involve palpable small nodules that are exquisitely tender to touch.

Notably, lower back soreness is generally not an early sign of pregnancy; it usually does not appear until after the third month of gestation. Conversely, lower back discomfort is common among many women in the days preceding menstruation.
