In the early stage of HIV infection, some individuals may develop an acute-phase rash on the skin surface. This rash typically appears approximately 2–4 weeks after viral entry into the body and commonly occurs on the face, neck, anterior chest, and back; it may also appear on the limbs and genitalia. In individuals with lighter skin tones, the rash may appear red or reddish-brown, whereas in those with darker skin tones, it may appear purplish or black.

What are the early skin symptoms of HIV infection?

During the initial phase following HIV infection, some individuals may develop an acute-phase rash on the skin surface. This rash typically appears approximately 2–4 weeks after viral exposure and commonly affects the face, neck, anterior chest, and back; it may also occur on the limbs and genitalia. In individuals with fair skin, the rash often appears red or reddish-brown, whereas in those with darker skin tones, it may appear purplish or black. The lesions are usually scattered, sometimes coalescing into patches, and manifest as macules, papules, or papulovesicles. They are generally small in diameter, typically non-pruritic, and occasionally associated with mild tenderness. With appropriate symptomatic management—or even without any intervention—the rash usually resolves spontaneously within 1–3 weeks.

This rash is non-specific and resembles viral exanthems caused by other viruses; therefore, it cannot serve as a diagnostic criterion for HIV infection. Even if a similar rash develops following high-risk exposure, self-diagnosis of HIV infection is inappropriate. Confirmation of HIV infection requires specific laboratory testing for HIV antibodies and/or antigens.
