The likelihood of becoming pregnant during menstruation is very low; however, the menstrual period is not necessarily a “safe period.” Intercourse during menstruation still carries a risk of pregnancy—particularly in women with very short menstrual cycles. In such cases, ovulation may occur shortly after menstruation begins. If the menstrual period is prolonged and ovulation occurs near its end, the chance of conception during menstruation increases.

Pregnancy occurs when a sperm fertilizes an egg in the fallopian tube. In a typical 28-day menstrual cycle, ovulation usually takes place around days 12–16—midway through the cycle—when one mature egg is released from either ovary into the abdominal cavity. This process is called ovulation. The egg is then rapidly drawn into the nearest fallopian tube through the fimbriated (tulip-shaped) opening at its distal end.

After ovulation, the egg remains viable in the fallopian tube for approximately 24 hours. Therefore, pregnancy can only occur if sperm are present in the fallopian tube during this brief fertile window.
